Portrait Application
Thank you for your interest in receiving a portrait of your fallen hero. We consider what we do a great privilege and we thank you for your interest in receiving a portrait of your loved one from us. In order to produce the highest quality portrait we can offer, artist Phil Taylor spends a great deal of time getting to know your hero's story, as well as those most important to them. With this in mind, we ask that you try and complete the application to the best of your ability. All information you provide is confidential and never shared outside our organization without your permission. We know this can be an emotional process, but the more information and pictures we receive, the better portrait of your hero we can produce.
Additionally, with a maximum of 20 portraits and presentations provided each year, our current backlog is significant. Please submit your application with this in mind.

Instructions before beginning:
- Portrait applications may be submitted by an immediate family member or a family representative.
- The original portrait is provided as a gift to the immediate family; ie. parent/s, sibling, spouse or child. In case of divorce or other special circumstance, the person completing the application can specify the immediate family member to receive the original. The original portrait is delivered framed, ready to hang.
- Dimensions: Portrait canvas is 24"x30"; with frame it is approximately 30"x36".
- Canvas reproductions and/or photo prints are available for those who did not receive the original painting, ie. other immediate family, aunts/uncles/cousins, military buddies, friends, dedications, memorials, etc. These can be ordered any time after original is completed.
- Photo prints (unframed): 5x7", 8x10", 11x14", 14x18"
- Canvas reproductions available (framed or as modern gallery wrap): 16x20", 18x20", 20x24", 22x28"
- Photo prints (unframed): 5x7", 8x10", 11x14", 14x18"
- Your application being accepted depends on the quality of photographs submitted. We are unable to paint from blurry or poor photos. However, if you are unsure what you have will work, we recommend completing the application anyway. We will get back with you if there is an issue and will try our best to help you get us the photos we need.
- Carefully review your application before submitting. Incomplete or incorrect information will delay or may disqualify your application.
- The acceptance of your portrait application is based on the guidelines listed, as well as funding limitations of our organization. A completed application does not guarantee a portrait will be produced.
